PLEASE READ THE FOLLOWING INSTRUCTIONS CAREFULLY

 

 The following conditions are required to complete this application:

 

  • You must be the account holder, or the account holder's agent, to make this application.
  • You must have the utility bill for which you are seeking a leak forgiveness. You will need to reference the bill in order to complete the application.
  • You must read, understand and agree with the Leak Forgiveness Policy before completing this application.

 

INSTRUCTIONS:

 

1. Read, understand, and agree with the Leak Forgiveness Policy. The policy is set forth under the authority of Municipal Code 16.13.050 and cannot be altered, expedited or otherwise deviated from by SCMU staff.

 

2. Answer the application questions completely. When asked for detail describing your leak, please provide as much information as possible.

 

3. Click on NEXT PAGE after answering every question on the page.

 

4. Click SUBMIT upon completing the application. If needed, SCMU will contact you to request further information. Allow 4 - 6 weeks for processing.

 

NOTE: During the time that the Leak Forgiveness is being processed, you must pay a normal bill amount for the bill(s) affected by the leak, as well as pay all subsequent bills, by the due date. Also, during the time that the Leak Forgiveness is being processed, you may receive reminder notices for the past due balance related to the leak charges.
